Program Overview

Adapt's Geropsychiatric Rehabilitation and Special Needs program provides 24-hour on-site supervision to adults with serious mental illness in a supervised housing program in Belleville, Illinois.

  • Summary of Services

    Adapt services are based on a psychiatric rehabilitation model in which each consumer is viewed in a holistic manner. Services are focused on improving each consumer's quality of life, acquisition of skills and integration within the community. This recovery-oriented approach emphasizes independent living skills, peer and family support, vocational development and consumer-centered goal setting.

    Adapt's Supervised Residential program is specifically indicated for consumers whose acuity level requires 24-hour on-site supervision. Adapt staff work directly on-site and provide services during all waking hours, as well as provide awake supervision during consumer sleep hours. Services are provided 365 days a year.  Crisis Intervention services to consumers and consultation with the psychiatrist are available as needed 24 hours a day.  Consumers are linked with community-based service providers including day treatment programs, local physicians and psychiatrists.

    An experienced, Masters Degreed level professional serves as the Program Director over the site, providing direction and supervision to program staff. The Supervised Housing program has a Team Leader, and sufficient Mental Health Professionals and Rehabilitative Service Associates to assure adequate supervision each day. Team Leaders and Mental Health Professionals possess Masters or Bachelors degrees.  Rehabilitative Service Associates possess a minimum of a high school diploma. A part-time Psychiatrist provides consultation as needed.
     
  • Admission Criteria

    Referrals into the program are overseen by the Illinois Department of Human Services – Division of Mental Health.  In accordance with DMH Prior Authorization and Utilization Review Guidelines, prior authorization by the Network Manager is required for entry to, and exit from, the Supervised Residential program. Referred consumers must have a diagnosis of severe mental illness, documented level of functioning impairment requiring clinical/rehabilitative supports, a clinically documented need for on-site 24-hour supervision and the capability of medication self-administration.

    Adapt will accept all clients referred by the Illinois Department of Human Services – Division of Mental Health or its agents without regard to race, religion, sex, national origin or handicap.  Referrals from other community sources must be screened and approved by Division of Mental Health liaisons. Acceptance into the program is also dependent upon the ability of the program to meet the consumer's needs. A consumer might be denied admission if his/her behavior presents significant risk to the health and safety of others.
