Skills Training and Development
Consumers are provided with opportunities to acquire and enhance skills needed to function effectively in their communities.
Independent Living and Supported Housing
Consumers living independently in their own homes are provided with support services by a treatment team. Support services incorporate evidence-based practices and include intensive case management, medication training and monitoring, illness management and recovery, crisis services, individual therapy, family supports, skill acquisition and support in role recovery.
Recovery Services
Consumers and their families are provided with individualized support services to assist them to work towards their personal goals of attaining meaningful roles in their communities and restoring their self-esteem and identity.
Counseling
Counseling is provided in order to resolve a concrete problem in daily functioning (problem focused, solution oriented) or relieve symptoms resulting from maladaptive thoughts, feelings, and/or interpersonal disturbances.
Medication Training and Support
Clinicians coach individuals on their diagnosis, medications, monitoring and management of symptoms and side effects.
Case Management and Linkage to Resources
Case management is offered to support consumers in identifying, acquiring and maintaining access to needed resources.
After-hours On-Call
Clinicians are available on-call 24 hours per day, 7 days per week to address the problems of individuals in crisis. Callers are provided with information, support and referrals to help them acquire safe and affordable services in the least restrictive setting possible.
Psychiatric Evaluation and Medication Maintenance
Board certified psychiatrists or nurse practitioners provide psychiatric evaluations and medication maintenance in outpatient settings for consumers diagnosed with severe and persistent mental illnesses. Outpatient psychiatric services include evaluation, development of a treatment plan, supervision of the administration of medications, monitoring of effects and side effects of medication and assessment of symptoms.
Assertive Community Treatment (ACT)
ACT is self-contained programming providing treatment, rehabilitation and support services to consumers with severe and persistent mental illnesses who have histories of multiple hospitalizations. Using an integrated services approach, the ACT team merges clinical and rehabilitation staff expertise, e.g., psychiatric, substance abuse or housing, within one mobile service delivery system.
Crisis Assessment and Intervention
Adapt offers telephonic and face-to-face mental health services aimed at assisting individuals or families in understanding and successfully resolving mental health crisis situations. Staff is available on-call at all hours, days, nights and weekends to address the problems of consumers in crisis with the goals being immediate crisis resolution, deflection from hospitalization, linkage with needed resources and maintenance of current community placement.
Dual Diagnosis Residential Program
The Dual Disorders Center at Wilmer, Texas is committed to the treatment of persons who are living with co-occurring disorders of significant mental illness and substance abuse disorders. Adapt treats each disorder as a primary diagnosis. The program provides residential treatment 24 hours per day, 7 days per week. Treatment is facilitated by a multi-disciplinary support team in a structured, therapeutic environment.
