Adapt of Illinois

Adapt of Illinois provides psychosocial rehabilitation for adults living in nursing homes, and supported housing services for adults living in the community. Services are provided in 5 counties in Illinois.

Community Support

    Skills Training and Development
    Consumers are provided with opportunities to acquire and enhance skills needed to function effectively in their communities.

    Independent Living and Supported Housing
    Consumers living independently in their own homes are provided with support services by a treatment team. Support services incorporate evidence-based practices and include intensive case management, medication training and monitoring, illness management and recovery, crisis services, individual therapy, family supports, skill acquisition and support in role recovery.

    Recovery Services
    Consumers and their families are provided with individualized support services to assist them to work towards their personal goals of attaining meaningful roles in their communities and restoring their self-esteem and identity.

    Counseling
    Counseling is provided in order to resolve a concrete problem in daily functioning (problem focused, solution oriented) or relieve symptoms resulting from maladaptive thoughts, feelings, and/or interpersonal disturbances.

    Medication Training and Support
    Clinicians coach individuals on their diagnosis, medications, monitoring and management of symptoms and side effects.

    Case Management and Linkage to Resources
    Case management is offered to support consumers in identifying, acquiring and maintaining access to needed resources.

Emergency and Crisis Services

    Crisis Assessment and Intervention
    Adapt offers telephonic and face-to-face mental health services aimed at assisting individuals or families in understanding and successfully resolving mental health crisis situations. Staff is available on-call at all hours, days, nights and weekends to address the problems of consumers in crisis with the goals being immediate crisis resolution, deflection from hospitalization, linkage with needed resources and maintenance of current community placement.

Residential Services

    Supervised Transitional Housing
    Consumers live in a 24-hour supervised group home setting and are provided with recovery oriented support services focusing on skill acquisition. With a goal of transitioning to a less restrictive setting, support services incorporate evidence-based practices and include illness management and recovery, medication training and monitoring, crisis services, intensive case management, in vivo skill acquisition and support in role recovery.

    Nursing Home Programs
    Consumers residing in a unique nursing home setting are provided with intensive services to support them in their living environment. Services are individualized and incorporate evidence-based practices. Services include skills building, medication training and monitoring, case management, illness management and recovery, crisis services, individual and group therapy, in vivo skill acquisition and support in role recovery. Intensive services are provided with the goal of supporting consumers in identifying and developing social, vocational, educational, and leisure skills that will enhance their lives and allow them to be successful in their living environments.

Adapt Community Alternatives

Adapt Community Alternatives provides independent and transitional living supports in the Chicago Metropolitan area, servicing young adults, ages 18 to 21. Adapt works independently and collaboratively with other agencies to provide tools for youth, formerly institutionalized or in foster care settings, to live successfully as productive citizens in their communities.

    Independent Living Support Services
    Community Support services allow young adults in care to live independently in the community of their choice. These services incorporate training in life and independent living skills, case management, and referrals for needed services --- including education, vocational, mental health, substance abuse and health care. Services take place in the community and in the home of the consumer rather than in an office.

    Interagency Collaboration
    In this special program, Adapt combines the strengths, abilities and expertise of two or more agencies to provide a unique array of services for consumers beyond that offered by any single agency. This program targets youth who are in need of support and services during the process of becoming independent, including those who are pregnant and parenting.

Transitional Living

    Transitional Living
    Adapt provides supported, group apartment living that allows young adults to have the experience of living in and maintaining an apartment with 24-hour support from clinicians. Transitional Living includes training in life and independent living skills and referrals for supportive services such as education and vocational services, mental health, substance abuse services, health care and case management.
