Nursing Home Programs
Program Overview
Adapt's Geropsychiatric Rehabilitation and Special Needs program provides specialized services to adult and geriatric individuals with serious psychiatric disabilities residing in nursing homes. Program locations are in Chicago, North Aurora, Rockford, Belleville, and Cobden, Illinois.
Summary of Services
Adapt services are based on a psychiatric rehabilitation model in which each client is viewed in a holistic manner. Services are focused on improving each client's quality of life, acquisition of skills and integration within the community.
Adapt staff work directly on-site and provide services for an average of 10 hours per day. Services are provided 365 days a year with a minimum 10 hours of staff coverage daily. Crisis Intervention services to clients and consultation with the nursing home are available as needed 24 hours a day. The integration of the nursing home services with Adapt's psychiatric services is the key to a successful placement for each client.
An experienced, Masters Degreed level professional, serves as Program Director at each site
providing direction and supervision to program staff. Each program has a Team Leader and sufficient Mental Health Professionals to assure that a minimum 1:9 staff to client ratio (on duty) is met each day. Team
Leaders and Mental Health Professionals possess Masters or Bachelors degrees. A part-time Psychiatrist provides on-site psychiatric services.
Admission Criteria
Referrals into the program are overseen by the Illinois Department of Human Services –
Division of Mental Health, and typically consist of individuals with severe mental illness who have been long-term residents of State Hospitals. First and foremost, Adapt clients are residents of the
nursing homes in which they reside. They receive services from the nursing homes, which would typically be provided for any resident. Adapt services are supplemental to the nursing home services and are focused on individualized psychiatric needs.
Adapt will accept all clients referred by the Illinois Department of Human Services – Division of Mental Health or its agents without regard to race, religion, sex, national origin or handicap. Referrals from other community sources must be screened and approved by Division of Mental Health liaisons.
Acceptance into the program is also dependent upon the ability of the host nursing home to meet the client's
needs. A client might be denied admission to the nursing home if the facility is unable to meet the clients care needs, or if the client's behavior presents significant risk to the health and safety of
others.
