Day Programs
Adapt provides Community Support services for persons with mental illness to support them in developing and maintaining their housing, employment and natural support systems. Each consumer works closely with a treatment team to define individual goals.

Skills Training and Development

Independent Living and Supported Housing
Consumers living independently in their own homes are provided with support services by a treatment team. Support services incorporate evidence-based practices and include intensive case management, medication training and monitoring, illness management and recovery, crisis services, individual therapy, family supports, skill acquisition and support in role recovery. Recovery Services
Consumers and their families are provided with individualized support services to assist them to work towards their personal goals of attaining meaningful roles in their communities and restoring their self-esteem and identity. Available from Adapt of Illinois, Adapt of Missouri, Adapt of Texas. |

Counseling
Counseling is provided in order to resolve a concrete problem in daily functioning (problem focused, solution oriented) or relieve symptoms resulting from maladaptive thoughts, feelings, and/or interpersonal disturbances. Available from Adapt of Illinois, Adapt of Missouri, Adapt of Texas. |

Medication Training and Support

Case Management and Linkage to Resources

After-hours On-Call
Clinicians are available on-call 24 hours per day, 7 days per week to address the problems of individuals in crisis. Callers are provided with information, support and referrals to help them acquire safe and affordable services in the least restrictive setting possible. Services for At Risk Youth
Community Support services allow young adults to live independently in the community of their choice. These services incorporate training in life and independent living skills, case management, and referrals for needed services --- including education, vocational, mental health, substance abuse and health care. Services take place in the community and in the home of the consumer rather than in an office. Available from Adapt Community Alternatives. Interagency Collaboration In this special program, Adapt combines the strengths, abilities and expertise of two or more agencies to provide a unique array of services for consumers beyond that offered by any single agency. This program targets youth who are in need of support and services during the process of becoming independent, including those who are pregnant and parenting. Jail Diversion Solutions
The Jail Diversion Solutions system is a public/private partnership between a community's criminal justice system, behavioral health organizations, state government entities, mental health providers, and HarrisLOGIC. The system uses data federation, instant messaging and online awareness to achieve unprecedented efficiency in Jail Diversion of non-violent offenders from county jails to community services. HarrisLOGIC works in partnership to ensure data for this solution and other envisioned workflows flow seamlessly in near real-time between the information systems of stakeholders.
