About Us

Services:
Pathways provides community support for youth ages 17 to 21 year-old wards of the Illinois Department of Children and Family Services to assist them to transition into adulthood and independence. Youth are assisted in the areas of employment, stable housing, education, financial self-sufficiency, parenting skills and independent living skills. Pathways achieves these goals through in vivo support, teaching and modeling in real-life roles.  The Pathways Program serves metropolitan Chicago

Eligibility for Services:
Youth must meet the following requirements to be eligible for Pathways:
•Be between the ages of 17 and 21
•Have the Illinois Department of Children and Family Services or a Guardian with court-ordered legal responsibility
•Have a permanency goal of Independence
•Have substantial need for mental health, rehabilitation and social services
•Have a documented history of difficulty achieving Service Plan Goals
•Need to learn responsible community living skills, vocational skills and educational skills
•Have required frequent emergency services in the past
•Have difficulty accessing community resources
•Lack a personal support network
•Express interest in apartment living
•State a commitment to complete educational and vocational goals
